Our dedicated process
Introduction
You (as the client, parent or family member) may come to us as a referral from a solicitor, deputy, an insurer, the NHS, or social services. Occasionally some clients come to us directly.
Initial Assessment
We organise a meet and greet to ensure the individual in need of Clinical Case Management is completely happy with the case manager who will take on their case. Once everyone is happy, we move onto assessing the required needs through discussions with the individual, their family, or carers. The aim of this is to evaluate the impact the injuries, and how we can use available resources to improve the quality of life and maximise potential.
Planning
We will write a detailed report following this assessment and present a tailored Clinical Case Management plan where we can explore the important values, prioritise goals, and identify the necessary therapies, support teams and finance required to achieve these.
Co-ordination
Following approval of the Clinical Case Management plan, we move towards the outlined goals. The dedicated Case Manager will co-ordinate all of the relevant services, professionals, and agencies, all whilst considering the most appropriate use of resources. During this time we advocate for the individual, proactively working on the prompt provision of services. Where required we will work to source fruther private provision to top up the use of statutory resources.
Monitoring
The case manager will regularly monitor progress and adjust the plans to meet goals and achieve the best results. We hold regular check-ins with clients, meet regularly with the multi-disciplinary team, schools, vocational providers and support teams to ensure goals are being met or exceeded.
Evaluation
This stage is ongoing where we evaluate the plans effectiveness and how well each service is working towards the goals, ensuring to address any areas that require further intervention. We also offer any fruther recommendations that might be necessary to reach the goals.

Types of cases we manage
Whether the case is unsettled or settled in litigation, we can help. We work closely and communicate effectively with solicitors and court of protection deputies to maximise quality of life and rehabilitation.
Where appropriate, we will set up employed or agency care packages to empower and manage daily activities.
Specialist additional support
We instruct private therapists, psychologists and other appropriate professionals to work upon areas that are important to help rehabilitation, promoting the ability to engage in activities which are meaningful for the individual.
We are registered with the Care Quality Commission (CQC) and have the necessary HR and administration support to effectively and safely implement bespoke care packages for you/your loved one.
